Oculo-orbital complications of sinusitis: A pros­pective study of 60 cases

Summary : Objective: To present the clinical, paraclinical, bacte­rio­logical and therapeutic aspects of oculo-orbital complications of sinusitis. Materials et metho­ds: A prospective study of 60 cases of oculo-orbital complicated sinusitis during a period from September 2010 to April 2014. Cases of endocranial complications were excluded. Results: The mean age of the patients was 31 years (02-60 years). A rhinopharyngeal infec­tion was the precursor to sinusitis in 42 cases (70%). Ophthal­mo­logic examination revealed 6 cases of blindness, 3 cases of periorbital fistula and 6 cases of exposure keratitis, one of which had evolved towards unilateral blindness. CT imaging showed 27 cases of pre-septal cellulitis (45%), 9 cases of orbital cellulitis (15%), 3 cases of subperiosteal abscess (5%) and 21 cases of orbital abscess (35%). 31 patients were mana­ged surgically; 18 cases had drainage of the orbital abscess, including 2 patients having a tarsorrhaphy, 3 cases had an endoscopic anterior ethmoidectomy with drainage of a sub­periosteal abscess and 9 cases a drainage of an abscess collection of the soft tissues. The post-operative period was marked by the persistence of a corneal opacity in 2 patients the persistence of an exophthalmos with paralysis of the CN3. Conclusion: Orbital complications complicating acute sinusitis are rare but serious. Management must be early and adapted; medical, surgical and preventive, which involves the correct management of rhinopharyngitis.
