Active bone conduction implant in a three year old pediatric patient with hemifacial microsomia: Case report

Summary : In the last decades, the pediatric population with hypo­acusis has benefited from the adaptation of new devices of bone transmission, active transcutaneous devices have been develo­ped, maintaining the integrity of the skin and the direct vibratory stimulation of the bone. There is no consensus as to the minimum age to carry out the surgery and it is a topic for continued debate. The limiting factors for adequate implan­ta­tion of these devices in children is the thickness of the cranial bones, necessary for adequate fastening without injuring adjacent structures and adequate thickness of the skin. We present a case of a male patient, 3 years and 2 months of age, diagnosed with hemifacial microsomia and bilateral atresia of the external auditory canal, facial asymmetry, macrostomia, patent foramen oval, and renal malformation. Despite making multiple adjustments to the bone conduction hearing aid, limited gain of function and delay in language development were observed in the patient. Therefore, it was decided to implant an active bone conduction hearing aid, MED-EL brand, Bone Bridge® model, in order to improve auditory perception, discrimination, and language. We present the results of treatment and follow up.
