Soft palate ventilation: Does the soft palate really exist?

Summary : Objectives: Helping to identify the role of the soft palate in the normal ventilation, and above all, in the pathological night ventilation (OSAS and snoring), and replace it into an regional anatomical set. Methods: We briefly summarise the history of OSAS and snoring treatments. Therapeutic failures are frequent when treating only the soft palate. In the light of recent practi­ces, we try to understand the reasons of thoses failures. Results: We purpose three ways of progress: Problems of scientific knowledge of the physician, of medical assessment at our dispo­sal (in particular DISE), and problems of representations models of the soft palate. Discussion: Many ENT physicians are now training to sleep diseases. The incorporation of scienti­fic knowledge and the wider use of the DISE, could help us to modify our mental representations of the soft palate. The normal soft palate ventilation function and the OSAS needs, to be understood, to be replaced into a large anatomical set.
